STEP ONE:
Click here to access the link to the Board website. You may be prompted to sign in.

STEP TWO:
Fill in the form and a digital copy will automatically go to your supervisor (Principal) and the Superintendent.
STEP THREE:
Keep a copy of the form for yourself in a safe place. You should be emailed a copy to your board email. You may also wish to print the form for your records.
STEP FOUR:
Call the District Office to let them know you have filled in the form (705) 726-1722
STEP FIVE:
If there is no follow up from a Board Representative in a week, contact the District Office
